Office Information: About Your Visit
 Insurance Information
We accept most major insurance plans. Please contact your insurance company prior to your appointment to make sure our physician is a participating provider with your plan. We do not participate in Medicaid.
Appointments
Please call the office to make an appointment in advance.
If you are unable to keep your appointment, please call us as far in advance
as possible so we may use that time to see another patient in need of care.
We will also call to confirm your appointment. We make a sincere effort to adhere to our appointment schedule; however, we appreciate your patience if we are late due to emergencies.
Fees & Payments
We will help in any way we can to assist
you in handling claims. Co-payments are collected prior to your visit.
Prescriptions & Renewals
Please request all new or renewal prescriptions during our normal business hours Monday through Thursday from 8:00am - 4:00pm. No analgesic/narcotic prescription will be renewed/filled at night or on the weekends.
